Field Name	Description
Username	Required. Your username will be your web account login. Your username must be unique in our system and may not contain sensitive information such as social security number.
Password	Required. Passwords must be 8-20 characters, no spaces or invalid symbols, 1 number, 1 lower and 1 uppercase letter, and not be the same as user name.
Confirm Password	Required. Must match what you entered for Password.
Phone Number	Enter a phone number where you would like to be reached in the event customer service needs to contact you regarding your account. Also select the country where the phone number is based.
Alt Phone	Enter an alternate phone number, if you have one. Also select the country where the phone number is based.
Email Address	Enter a valid email address. You must have access to this email to verify and complete the set up. Having an email address on file will allow you to use the Forgot Password reminder in the event you cannot remember your password.
Date of Birth	Enter your legal birth date. This value is used to confirm your identity.
Address	Provide your residence address.
Mailing Address	If you use a mailing address that is different from your home address, provide it. Otherwise, check "Same as primary address above."
Electronic Communications Disclosure	Accept the electronic communications disclosure.

Submit the form and your will see the “Congratulations. You have completed account set up.” message.

Check your email and click the link to complete set up. If you did not enter your email address, you may click “Click Here” on the confirmation page to login with the username and password you just set up.
