If you have a card with us...

Your employer will issue a card for you, or you may have a gift card. You must activate the card using either this online account set up or the automated telephone system via the phone number shown on your card.

To activate your card online, start by following these steps:
  • Enter Card number: Enter your Card number in the box provided. Do not include spaces or dashes when you enter the number.

Click "Submit" to continue creating your account.

If you do not have a card with us...

Please provide the information listed below. If you are uncertain about any of the values that should be provided, please contact your employer for assistance.

  • First Name: Your first name, as it appears in your records with your employer.
  • Last Name: Your last name, as it appears in your records with yoru employer.
  • Unique ID: The unique value, such as social security number or employee ID, which your employer has used to identify you when creating your account. If you are unsure which value to use, please contact your employer.
  • Employer Name: The name of your employer. You should have been informed regarding what value to provide in this field.
